Mitchell and Area Community Outreach is a community support agency providing service to enable seniors, disabled and those recuperating from injury, illness and accident to remain living independently in the Municipality of West Perth. Mitchell and Area Community Outreach operates under the umbrella of West Perth Village, Mitchell Ontario.

Your financial gift will help to keep programs affordable and enhance the quality of life for those living in the Municipality of West Perth. Financial gifts can be mailed or taken to the Outreach Office. Tax receipts are issued for these gifts.

To register for one of our various programs please Contact Us today. For most events, at least one week’s notice, prior to the event date, is required to guarantee you a spot. To note, some events have deadlines response dates. Please take a look at our Calendars for upcoming events. New clients will have to answer a few questions prior to being able to attend any event for health and safety purposes.

All new clients will go through an intake process. New clients should call central intake at 1-844-482-7800
Neighbourly Services programs designed to help individuals remain living independently in their home. These programs include:
Volunteer Transportation, Bathing Program, Friendly Visiting, Grocery Assistance, Home Help/Home Support, Let’s Go Home (LEGHO)
Friendly Visitors are volunteers who are interested in visiting with clients who live in their own homes and would like some extra socialization. Volunteers provide companionship and support to help individuals retain their independence.
A Friendly visitor can visit in a client’s home, play games, go for a walk, visit at the local coffee shop or activities of mutual interest to the volunteer and client.
There is no fee for this service as it is provided by a volunteer
Volunteers are matched with clients based on factors such as location, similar interests and personalities. We try very hard to make sure volunteers and clients will be compatible. Careful screening is used when matching volunteers with clients. All volunteers go through a police screening process as well as reference checks and interviews.

Therapeutic baths can be scheduled weekly or as needed at the West Perth Village in the hyrdosonic tubs. Baths are done by trained healthcare staff.
Rates: $30.00/half hour
(a $30.00 cancellation fee is charged if baths are not cancelled the day before)
The Hydrosonic tub is a low frequency ultrasound which propagates pressure waves in water. These waves continuously create millions of microscopic bubbles and remove dirt particles from the client’s skin, gently and effectively. The Hydrosound’s gental massaging action offers a new standard of cleanliness without discomfort. The benefits of the bathing program hydrosonic tub are:
- personal hygiene
- promotes healing of wounds
- improves circulation
- a safe alternative to bathing for those who need assistance

Groceries are ordered by the client calling Mitchell Outreach (519-348-9875) on the Friday, before noon. The volunteers pickup the client’s list and money to go shopping and then returns to deliver the groceries to the client’s home. Groceries and delivery fee is paid directly to the volunteer when ordering groceries.
Orders must be placed by 12:00pm on the Friday to receive groceries on the following Tuesday.
Fee: Dependent upon location of delivery (approx. $10.00)

Mitchell & Area Community Outreach works in partnership with ONE CARE Home & Community Support Services to provide Home Help/Home Support and Let’s Go Home Services. Call our office at 519-348-9765 for more details.

Home Help/Home Support
This program provides assistance with household chores and daily tasks such housekeeping, laundry, in-home meal preparation to name a few. Respite care can also be provided when the family care-giver needs a break. Work done is individual to the person’s needs.
The cost varies depending on the length of time utilized
Let’s Go Home (LEGHO)
This program provides clients access to free community services for a short period of time to ensure a successful recovery period so they don’t return to the hospital. A Care Planner does an assessment and regular check ins, and links clients to various services they may need. In addition to Care Planning, Transportation, Meals on Wheels, and Home Help are the core services provided to all clients. These services provide support while clients rebuild their strength and ability to live independently. This is not only crucial for our clients, but for the health system as well as it helps to relieve pressure in our hospitals.
What is included in the Let’s Go Home program? You will receive Care Planning, support which includes weekly follow-up and intensive care planning to begin within 72 hours of discharge for a period of four to six weeks. This includes information and linkages to other services to help you and your family as you recover.
In addition, you must require at least two additional services such as:
- Transportation and assistance with your ride home, including help from an attendant to settle at home on the day of your transition from hospital.
- Up to two weeks of Meals on Wheels/frozen meals delivered to your home.
- Up to two free transportation rides to medical/follow-up appointments within six weeks of transition to your home.
- Up to three free hours of Home Help within four to six weeks of discharge.

To become a volunteer there are a few items that need to be completed. Interested individuals are required to complete an application form, provide 2 references and complete a criminal record check. A confidentiality form will also have to be signed. The confidentiality form ensures that all volunteers are aware that all matters pertaining to the operations of Mitchell & Area Community Outreach or personal information pertaining to the clients, members or families involved with Outreach is kept private and confidential. There will also be an interview with the Volunteer Coordinator.

Delivered Monday, Wednesday & Friday between 11:45am-12:30pm. Meals are not delivered on statutory holidays.
- Nutritious meals are delivered by volunteers to individuals who need assistance with their nutritional needs
- Each meal contains a protein, starch, vegetables, dessert and a drink for $11.00
- Deliveries can be arranged for one or multiple days with the option of multiple meals each day
- Meals are tailored to meet special diets, allergies and personal preferences
- Special therapeutic diets and texture modifications are available
- Three week menu cycle rotation
- Billed monthly
- Hot meals are prepared by the dietary staff at West Perth Village
- Peace of mind for families as agency follows up with client absence
Delivered on a two week rotation schedule.
- Single Entree: $7.75
- Soup/Dessert: $3.25

Apetito frozen meals are also available to supplement hot meals or as an alternative. These are great to store in the freezer for future use. Check out the Apetito Website for more information on their products.

Meal Requests
To request a meal you must call the Outreach Office at 519-348-9765, at a minimum, before 9:30 am on the date of the delivery. You are not able to make same day requests after this time.
Meal Cancellations
To request a meal you must call the Outreach Office before 9:30 am on the date of the delivery. Any meals that are not cancelled by this time will be charged, even if it is returned. If you decide you no longer need the service and cancel before 9:30am there is no charge and you will be reimbursed in full for any meals that you had prepaid.
Meal Costs

All groceries will be purchased at the local Valumart in Mitchell. Volunteers will be shopping on your behalf. Orders must be placed by 12:00PM on Friday to receive groceries on the following Tuesday. Items must be paid by cash or prepaid PC Gift Cards. Options to order and pay online may be available.

West Perth Transit is a door-to-door transportation service for seniors, the disabled and those recuperating from injury, illness and accident in the Municipality of West Perth. This service has been in operation since 1985 and now has 2 mobility vehicles to meet client needs. One vehicle is up to 16 passengers bus and the other is an accessible mini-van

Mitchell & Area Mobility Bus History
Mitchell & Area Mobility Bus began transporting clients in West Perth in 1985. It started as a result of a seniors’ needs study conducted in 1984 which indicated a need for specialized transportation services. The service was officially launched on October 1, 1985 with one service day.

Waiting fee of $20.00/hr will be charged once the client is dropped off until pickup time
If the trip is one way only for the client, the return kilometer rate will be charged
Charter Rates
$50.00/hr for driving
$40.00/hr for waiting/loading/standby
There is a one hour minimum charge of $40.00 for all charters booked for less than one hour
For West Perth residents traveling within West Perth – $40.00/hr for both driving and waiting/loading/standby

Special charter bookings can be arranged outside daily hours of operation.
To book transportation, call Easyride at 519-272-9875 from 8:30am – 4pm. 24hours notice for booking is required but last minute appointments may be accommodated.
Cancellation policy – Cancellations should be made as soon as possible to accommodate others. A one way rate charge will apply to cancellations at time of pickup or if a client is not home at the requested time of pickup.

Volunteers provide transportation to clients for out of town medical appointment. Outreach staff will match clients with volunteers who will provide the trip.
Clients pay a fee to the drivers which reimburses them for their fuel. Reimbursement is based on a per kilometer fee. Outreach Office staff will inform the client about the full cost of the trip when the staff call back to confirm that a driver has been booked. Clients must pay the driver directly at the time of pickup for the trip. If there are any parking fees incurred, the client will also pay for them at the parking area.
Clients are required to inform staff of any extra stops that need to be made when booking their trip, so that the cost can accurately be calculated. Unless it is a medical necessity (i.e. stop at the pharmacy), the driver will not make any extra stops that have not booked ahead of time. If there needs to be a stop for medical reasons that has not been booked prior to the trip, it will be at the discretion of the driver if they will be able to accommodate the stop. The client will have to pay $3.00 at that time for that stop. As well, if the client lives outside of Mitchell, there will be an extra $3.00 for picking fee in the rural area.
At no time is the client to call the volunteer at their home. If a booking needs to be changed or cancelled or a trip added, the client must call Easyride.
If the volunteer arrives for trip pickup at the client home, and the client is not home or denies the trip at that time, the client will be billed for the trip.

The initiative began in 2008 consisting of seven community support agencies providing services throughout their respective counties. Wellness for Seniors (WFS) of Perth and Huron has been funded through the South West Local Health Integration Network (LHIN) to create a wellness program based on the enhancement of existing programs and the creation of new ones. The WFS programs bring wellness, education and health monitoring to community dwelling individuals in rural and isolated areas. Promoting and sustaining improved health for older adults through a standardized best practice model of programs in local communities across Huron-Perth. All services will be provided with the overarching themes of education, exercise and health promotion/monitoring. WFS programs encourage individuals to manage their own health. Wellness programs have been created to provide a safe, inexpensive avenue for individuals to remain active and healthy.
The use of volunteers for program delivery allows for a high volume of programming with low costs of delivery. All seven agencies partnering in this project have collaborated in program delivery and development, along with community partners including Family Health teams, Family Physicians, Health Units, Community Professionals and other agencies.
